However, a much less common group of hair loss that is more complex is Scarring hair loss or cicatricial alopecia. In this condition the hair follicle is permanently damaged and replaced with scar tissue.
Scarring alopecia is of two types. Primary scarring alopecia where hair follicle is the main target of destruction and Secondary scarring alopecia where destruction of the hair follicle is incidental to some injury or pathological process, such as severe infections, burns, radiation, or tumors.
In this condition, sometimes the hair loss may be slow and gradual, a diffuse non descript thinning. While other times pain, redness or trichodynia may be part of symptoms.
Proper medical history, hair and scalp examination and trichoscopy are essential to narrow down the diagnosis. In absence of a tell tale history like lupus arthritis, scalp burn or radiation etc; a scalp biopsy is recommended.
Treatment is directed towards the cause. Anti-inflammatory medicines like corticosteroids, hydroxychloroquine, cyclosporine etc are useful for lichen planopilaris, pseudopelades , frontal fibrosing alopecia and central centrifugal alopecia. Oral and local antibiotics are used for folliculitis decalvans and tufted folliculitis.
Surgical treatment for cosmetic benefit is an option in some cases after the disease has been inactive for one to two or more years. Hair restoration by transplantation or scalp reduction surgery may be done by a qualified plastic surgeon in these cases.
